NEW CLIENT INFORMATION
If you are a new client and have already set up your first appointment, we will need some additional information before you come in.
Please download, print and complete the intake form and then fax it to our Administrative Office at 410.561.9587.

HIPAA
Please also review the Notice of Privacy sheet (included on the intake form and on the HIPAA page) which provides information from the Health Insurance Portability and Accountability Act (HIPAA) about privacy and your health information. A longer, more detailed version of this information is available from our administrative staff in the Timonium office.
Feel free to bring questions about any of these forms for discussion in your first counseling session. We'll ask that you sign that you have read, understood and had opportunity to discuss all of this information. Thank you.
